Please note that due to the high volume of patients we see daily, we do not see walk-in patients. If you need an unscheduled visit, please call the Urgent Care Line, (405) 702-9777 or (405) 948-4900 ext. 219. This line is open at 7:45 a.m. every weekday except Wednesday.

If you need immediate treatment, go directly to an Emergency Room.

If you are a current patient and would like to make an appointment at our Clinic, please contact the Oklahoma City Indian Clinic Appointment Desk at (405) 948-4900 ext. 261 or 259. 

If you are a new patient and would like to make an appointment at our Clinic: To establish a chart at the Oklahoma City Indian Clinic you must complete a registration packet and have a new patient physical before additional services can be rendered. The Registration Clerks accept telephone calls to schedule new patient physicals the first Wednesday of each month between the hours of 7:45 a.m. to 11:00 a.m. We currently have a waiting list for new patient requests and are working diligently to process as many requests as possible each month.  For more information on our new patient process, please call the Registration desk at (405) 948-4900 ext. 221, 222, or 233.

New Patients and “Lapsed Patients”:
If you are a new patient, or a patient who has not been to the Clinic in at least one year, you must be at our facility one hour in advance of your scheduled New Patient Physical appointment. If you do not come in for your appointment one hour in advance, you will forfeit your appointment. If you miss your appointment you will not be allowed to re-schedule another New Patient Physical for at least six (6) months. Thank you for your understanding in this matter.

Required Documentation:
To establish a chart at the Oklahoma City Indian Clinic you must:

  • Bring all required ORIGINAL documents listed in the Document Review Form;
  • Have a new patient physical;
  • Complete a registration packet (all items linked below)

You may pick up a registration packet at our facility or for your convenience you may print and complete the required forms linked below. All registration forms must be completed before you arrive for your new patient physical appointment. Click each link below to print and complete the forms necessary for your appointment.

Payment for Services:
OKCIC is required by Federal Law (Indian Health Care Improvement Act Amendments of 1988 PL 100-713) to seek and collect payment from any medical program that the patient is entitled to participate in. This means that if you have insurance, the clinic is required by law to bill your insurance and if you are eligible for coverage such as SoonerCare, we are required to assist you with the application process.

The clinic works very hard to seek out and win grants that enable us to expand our services and meet the needs of our patients. This is why we need the partnership of our patients to take advantage of health care programs you may be entitled to and help increase the health care opportunities available at the clinic.

OKCIC has two Benefits Coordinators on staff that are responsible for providing education and enrollment assistance for various programs such as:

  • Medicare (including New-to-Medicare counseling and Medicare Prescription Drug coverage)
  • Benefits associated with the Oklahoma Health Care Authority including SoonerCare, SoonerCare Choice, SoonerPlan, SoonerCare Supplemental, TEFRA, & Insure Oklahoma
  • Aging Services
  • Veterans Benefits
  • Social Security disability services
