Community Health Representative (CHR)
(Travel Assistance) Program
Many of our clinic patients, who are most in need of our services, may have difficulty with transportation. We never forget that we are one community, and don’t want anyone in our community to go without needed health care services because of lack of transportation or the difficulty of using public transit.

To be sure that all of our clients get the care they need, the Community Health Representative (Travel Assistance) Program provides transportation to and from the clinic for those with scheduled appointments.

To use this service you must be:

  • A registered patient at OKCIC, or the parent of a minor child who has an appointment
  • You must have an appointment booked between 10AM and 2PM.
  • You must live in Oklahoma County.
  • You must call and arrange transportation at least two weeks before your appointment.
  • You must call and confirm 24 hours before you are to be picked up, or we will not come.
  • If you need to cancel, you must cancel at least 24 hours in advance, or transportation in the future may not be possible.
